DENTAL TREATMENT WARRANTY

Creekside Dental, Inc. openly communicates how we stand behind our quality and workmanship. We are proud of our dental services and what they do for our patients. Our goal is not to overly correct your dental problems, but have the work we do, last for many years into the future, to save you time and to save you from unnecessary expenses.

The long term success of our dental treatment for you is dependent on: How well you care for your teeth, eating a sensible diet and adhering to the schedule we set for you for the frequency of your professional examinations, cleanings and fluoride treatments. The products we recommend for you and the frequency of professional continuing care visits depends on your individual situation.

Your prescribed hygiene recare appointment frequency is every 3, 4, or 6 months.

Recurrence of decay is NOT covered by this warranty and is prevented by proper homecare, having your teeth cleaned regularly and using anti-cavity medications as prescribed.

DENTAL TREATMENT WARRANTY

Creekside Dental, Inc. offers a Dental Treatment Warranty (“Warranty”) for your peace of mind. Our warranty provides protection for you and is most effective for those patients who participate in their dental health by (1) following our recommendations for treatment and (2) staying consistent with their re-care visits. This Warranty is effective upon completion of the Warranty Service Item, and your Warranty coverage is outlined, as shown in the tables below.

MINIMUM WARRANTY REQUIREMENTS

Your Warranty provides the most effective coverage if you satisfy each of the Minimum Warranty Requirements below. If you do not satisfy these requirements, you may still have Warranty coverage, but such coverage will be limited as provided herein.

  • Return for re-care visits as prescribed above, not more than fifteen (15) days past your re-care due date
  • Use any and all anti-decay or gum disease medication as prescribed
  • Follow our recommendations regarding destructive habits such as clenching and grinding, which may fracture your teeth
  • Keep account in good standing with our office, such that it has not been placed with a collections agency
  • (For full upper and lower denture patients) Have re-care visit at least once every twelve (12) months
  • (For denture and partial patients with natural teeth remaining) Have re-care dental visit at the frequency prescribed above

* Defective workmanship shall be determined by the dentist’s sole discretion.

GENERAL EXCLUSIONS : Your Warranty does not cover damage to sealants or damage to teeth or prosthesis caused by accidents, trauma, negligence or improper use (for example: grinding, clenching, chewing ice, or biting non-food items). Your Warranty also does not cover recurrence of decay, which is prevented by proper homecare, having your teeth cleaned regularly, and using anti-cavity medications as prescribed. For dentures, partials and flippers, the Warranty does not cover damage caused by the physical mishandling of the prosthesis, such as the dropping of the prosthesis or the prosthesis being chewed by a pet. Denture or partial realigns are not covered by this warranty.

WARRANTY PROCEDURE : To make a claim under this Warranty, simply indicate that you desire to make a Warranty claim when you schedule your in-office appointment. When scheduling your appointment state the nature of the problem. At the time of your visit, the dentist will determine whether or not the issue you identified is covered under this Warranty and will advise you of same. If your claim is covered, the repair or replacement will either be performed immediately, or scheduled for a more appropriate day and time. In the event that a prosthetic needs to be replaced, please allow adequate time for a replacement to be made and delivered.
